Referrals and forms

All referrals should be sent using the Electronic Referral System (ERS) using the standard MSK Sheffield Referral Proforma. 

Please choose either Orthopaedics or Physiotherapy on ERS:

Orthopaedics – Foot and Ankle – MSK Sheffield
Physiotherapy – MSK – MSK Sheffield Foot & Ankle

All referrals will be triaged by a Consultant and/or Senior Therapist to direct the patient to the right service 

Please ensure if imaging is requested that foot and ankle x-rays are weight bearing.
